Why Does Uchiha Madara wear a mask?

Possible reasons:
  • Nothing / No reason
  • To hide his identity
  • Uchiha Madara must like fashion LOL
  • Kishimoto hide the left eye for us to think that it was Uchiha Obito. Remember that Obito gave his left Sharingan to Kakashi.
  • It has something to do with the secrets of his left eye? But it was revealed that he has two Sharingan. He used the left eye when he performed Izanagi and lose his sight. Later on he put the Rinnegan eye on his left.

The Mask with Distinctive Flame Pattern

This is Uchiha Madara's Mask 12 years ago when  he attacked Uzumaki Kushina giving birth to Naruto. Notice the distinctive flame patter. Also this is the mask he wore when he is approached by Uchiha Itachi.

Tobi/Uchiha Madara's Spiral Pattern Mask

Appearing as Tobi on Akatsuki, Uchiha Madara wear this mask. As you notice, he sucks up everything in his right eye. Even when he transported Sasuke and Karin.

The New Mask

As you can see it has a Sharinga design on a spiral pattern mask making it look like a Rinnegan over a Sharingan. He wear this mask when  he obtained Rinnegan on his left eye.

Whenever Madara acquired new power, he change his mask. Whatever be the reason, we don't know it yet. Let us wait till he show his face behind that mask. On the mean time read this article about The Real Identity of Uchiha Madara.
